1. Introduction
Lift Family Care ("we," "our," or "us") is committed to protecting the privacy of our employees, caregivers, and the individuals and families we serve. This Privacy Policy explains how we collect, use, disclose, and safeguard information through our Lift Care App ("App"), which is used to manage non-medical home health care services for individuals with disabilities. The App collects and processes Health Data, as described in detail in this policy, to facilitate care coordination and service delivery.

- Account information (name, email address, phone number, authentication credentials)
- Employment information (role, certifications, work schedule, pay rates)
- Communication records within the App (messages, incident reports)

- Care recipient demographics: name, date of birth, gender, physical descriptors, emergency contacts, and legal guardian/authorized representative information
- Care plans and goals: individualized care plans, therapeutic goals and objectives, protocols, and progress tracking

- Shift and care documentation: caregiver shift notes describing care activities, goal/objective progress assessments with performance scores, support levels provided (independent, verbal prompts, physical assistance), and outcomes
- Incident and safety reports: documentation of incidents including falls, injuries, behavioral events, near misses, and environmental hazards, with severity ratings and follow-up notes
- Delegation of care tasks: records of clinical tasks delegated to caregivers, including delegating clinician identity and authorization periods
- Health-related documents: uploaded care plans, health assessments, and other documents relevant to the individual's care
2.3 Location Data
- GPS coordinates when a caregiver clocks in, clocks out, switches service, or returns from a break (to verify the visit for electronic visit verification)
- One location reading when the App clocks a caregiver out automatically at a care limit set by their agency (see Section 8)
- Service delivery location coordinates (for electronic visit verification compliance)
- Mileage trip data including origin and destination addresses and coordinates (for transportation reimbursement)
2.4 Information Collected Automatically
- Device information (device type, operating system, app version)
- Usage data (features accessed, time spent in app)
- Log and diagnostic data (errors, performance metrics, crash reports)
- Website analytics from third-party providers, including aggregate traffic measurement and product-analytics tools that record on-page interactions such as pages viewed, clicks, and scrolling in order to help us improve the site. Information you type into our interest form is submitted through a separate secure embedded form and is not captured by these analytics tools.

To exercise these rights, please contact us using the information below.
8. Location Services
The App uses your device's location for electronic visit verification (EVV), for automatic clock-out at a care limit, and, where your agency uses it, for mileage reimbursement.
Clock-in and clock-out. When a caregiver clocks in, clocks out, switches service, or returns from a break, the App takes one location reading while the App is open.
Automatic clock-out. On visits that require EVV, the App asks for permission to use location in the background ("Always" on iPhone, "Allow all the time" on Android). It uses this for one purpose. When your agency has set a limit on a visit, such as a client's authorized hours, a daily limit, or overtime, the App keeps running in the background so it can clock you out at that limit, and it takes one location reading at that moment. On iPhone, the App receives low-accuracy location updates in the background so it can keep running. These are not saved or sent to us. Apart from these readings, and trip locations you choose to enter for mileage, the App does not record your location during a visit.
Who can see it. Your agency can see these readings. The client and their family can see where a visit started and ended. On EVV visits, the readings are sent to the state's EVV system, as described in Section 4. We do not sell location data or use it for advertising.
You can change location permissions in your device settings at any time. Without location, a visit that requires EVV cannot be verified automatically, and your agency may need to approve it by hand.
9. Changes to This Policy
We may update this Privacy Policy from time to time. We will notify you of significant changes through the App or other appropriate means. Your continued use of the App after changes become effective constitutes acceptance of the updated policy.
